Primary care for the whole family providing comprehensive and continuous care including prevention, diagnosis, and treatment of acute and chronic illnesses, health education, and disease prevention. 

Preventative care and wellness focus on maintaining health and preventing illness before it occurs. This approach includes routine check-ups, screenings, immunizations, and counseling to identify potential health issues early and reduce the risk of developing chronic conditions.

Diabetes Self-Management Education and Support (DSMES) equips nurses to educate patients on managing T2D, leading to better health outcomes. Patients will understand the T2D care and disease process. It will inform stakeholders on measures to promote patient satisfaction and improvement in treatment plans. Better sugar control can potentially reduce the risks of macro and micro health complications derived from T2D, reducing healthcare costs.

Our women’s health services evaluate a woman’s overall health, including medical history, and lifestyle. We provide all the recommended prescreening testing to promote early detection and intervention as needed.
